Whether you are a new patient at Comprehensive Pain & Neurology Center or have been with us for years, here is all the information you need to make the most of your visit as we work together to help you relieve pain and reclaim your life.
New Patients
What do I need to bring to my appointment?
Please bring the following to your first appointment:
- New patient appointment forms
- Current insurance card
- Driver’s license or state-issued identification
- List of current medications
Are there forms I need to complete?
Yes! Please complete these forms. You can either download them and fill them out by hand, or complete them on your computer and print out.
When should I arrive?
Arrive 45 minutes prior to your scheduled appointment or procedure time.
If you are scheduled for a procedure: Carefully review the Patient Procedure Instructions prior to arriving for your scheduled procedure as arrival times may vary.
Do I need anything from my referring provider?
Although the following should be sent by your referring provider, in order to serve you best, please also bring copies of the following:
- Recent medical records
- Most recent imaging reports (X-rays, MRI or CT scans)
- Electromyography (EMG) or Nerve Conduction Study (NCS) results
Current Patients
When should I arrive?
Patients scheduled for a procedure: Arrive 30 minutes before your scheduled procedure time. Be certain to carefully review the Patient Procedure Instructions prior to arriving for your scheduled procedure.
Follow-up clinic appointments: Arrive 15 minutes before your scheduled appointment time.
Are there forms I need to complete?
Yes! Please complete the forms listed below. You can either download them and fill them out by hand, or complete them on your computer and print out.
